چکیده

Methods NEO-ESCAPE was a randomised two arm phase II study in inoperable ovarian cancer designed as an external pilot to inform a future phase III randomised controlled trial. The primary objective was to assess the feasibility of two new extended chemotherapy regimens with 44 patients required on each arm. If one or both treatment arms proved feasible, the trial would continue to recruit to the feasible treatment arm(s) to improve the estimates of outcomes to be used in the phase III trial. Stopping rules were essential to enable pre-planned decisions on the futility of continuing to the required 44 patients on each arm. Simulations to assess futility were used in the decision making process.

Operational challenges and solutions with implementation of an adaptive seamless phase 2/3 study.

A wide variety of operational issues were encountered with the planning and implementation of an adaptive, dose-finding, seamless phase 2/3 trial for a diabetes therapeutic. Compared with a conventional design, significant upfront planning was required, as well as earlier, more integrated cross-functional coordination. The existing infrastructure necessitated greater flexibility to meet the nee...

A phase I/II clinical trial for adult recurrent glioma using 131i-tm-601, an iodinated peptide derived from scorpion venom

131I-TM-601 is a 36-amino acid peptide, called chlorotoxin (TM-601), derived from scorpion venom labeled with I-131. TM-601 binds a receptor on the surface of tumor cells, and not on normal cells. A single dose of 131I-TM-601 administered intracranially to human xenografted mouse models of glioma has been shown to extend survival up to 269% in multiple studies. 131I-TM-601 is in a multi-center ...

Estimation bias in survival data within clinical trials that use adaptive seamless designs

Adaptive designs are utilised in late phase clinical trials due to their efficiency in answering multiple clinical questions through a single trial. For example, seamless phase II/III clinical trials are used to answer phase II and III objectives via a single trial with two stages, where stage 1 and 2 represent phase II and III components respectively. At the end of stage1, an interim analysis ...
